Choosing between ureteral access sheath sizes looks simple on a price list: two numbers, a length, and a pack count. In the operating theatre it matters a lot more. The size decides which flexible ureteroscope will pass, how well irrigation fluid drains back out, and how easily the sheath goes up the ureter. This guide explains what the numbers mean, which sizes and lengths are most common, and what hospitals and distributors should check before they order.
What a Ureteral Access Sheath Does in Flexible Ureteroscopy
A ureteral access sheath is a thin, reinforced tube with a tapered inner dilator. It is passed over a guide wire, usually under fluoroscopy, from the bladder into the ureter. When the dilator is removed, the outer sheath stays in place and gives the flexible ureteroscope a smooth, open channel to the kidney.
The sheath does not treat anything by itself. Its value is practical: the scope can be taken out and put back many times during one procedure, for example to remove stone fragments, and fluid can flow out around the scope so the view stays clear. A smooth inner lumen also reduces wear on expensive flexible scopes. You can see our full range of ureteral access sheaths in different diameters and lengths.

How to Read Ureteral Access Sheath Sizes (Fr)
Sheath sizes are written in French (Fr), where 1 Fr equals about 0.33 mm. Every sheath has two numbers:
- Inner diameter (the first number): the working channel. It must be larger than the outer diameter of the ureteroscope.
- Outer diameter (the second number): the part that sits in the ureter. A smaller outer diameter is easier to place; a larger one gives more room inside.
So a 12/14 Fr sheath has an inner diameter of about 4 mm and an outer diameter of about 4.7 mm. The gap between the scope and the inner wall is what lets irrigation fluid drain back out, so the inner size is usually chosen to leave a little space around the scope.
Common Sizes and Lengths at a Glance
Most manufacturers offer a similar set of diameters. The table below shows the sizes hospitals ask for most often and where each one is typically used. Final selection is always the urologist’s decision.
| Size (inner / outer) | Approx. outer diameter | Often chosen for |
|---|---|---|
| 9.5 / 11.5 Fr | 3.8 mm | Narrow ureters and smaller-diameter scopes |
| 10 / 12 Fr | 4.0 mm | Routine flexible ureteroscopy |
| 11 / 13 Fr | 4.3 mm | Standard scopes with extra outflow space |
| 12 / 14 Fr | 4.7 mm | Larger stones and more fragment removal |
| 14 / 16 Fr | 5.3 mm | Pre-stented ureters and larger scopes |
Choosing the length
Length depends on anatomy. Shorter sheaths, roughly 20 to 28 cm, are commonly used for female patients. Lengths around 35 to 36 cm and 45 to 46 cm cover most male patients, and 55 cm versions exist for taller patients or special cases. Many hospitals simply stock one shorter and one longer length in their two most-used diameters.
What to Check Before You Order
Once the size range is clear, quality is what separates one sheath from another. These are the points our customers ask about most:
- Hydrophilic coating. The outer surface should become slippery when wet and stay that way through the procedure.
- Kink resistance. A coil-reinforced or braided wall helps the sheath keep its round shape in a curved ureter.
- Dilator fit and taper. The dilator should lock firmly into the sheath hub and have a smooth, gradual tip.
- Radiopaque markers. A clearly visible tip and markers make positioning under fluoroscopy easier.
- Packaging and labelling. Sterile, single-use packs should show size, length and lot number clearly on the outside.
- Certification. Ask for ISO 13485 certification and relevant registrations such as CE or CDSCO.
Sizing Tips for Hospitals and Distributors
Stock planning is easier when it follows how your urologists actually work. Start by listing the outer diameters of the flexible scopes used in your theatres, then match each scope to the smallest inner diameter that still leaves room for outflow. For most departments this narrows the choice to two diameters.
Next, look at past usage. If most procedures are for kidney stones in adult patients, 10/12 Fr and 12/14 Fr in 35 cm and 45 cm lengths cover a large share of cases. Keep a small number of 9.5/11.5 Fr sheaths for narrow ureters. Distributors supplying several hospitals often add 11/13 Fr and a 55 cm option to cover different preferences.
Access sheaths are usually used alongside other disposables in the same procedure, such as hydrophilic guide wires, stone baskets and DJ stents. Buying them from one manufacturer of urology medical devices keeps quality, documentation and delivery in one place.
Custom and OEM Ureteral Access Sheath Sizes
Some distributors and brands need sizes or lengths outside the standard range, or want sheaths packed under their own label. SPG Medisafe manufactures access sheaths in India under ISO 13485 controlled processes, with assembly and inspection in a cleanroom before sterilisation. We support custom diameters, lengths, coatings and private-label packaging for partners in India and abroad.
Ureteral Access Sheath Sizes FAQs
What does 10/12 Fr mean on a ureteral access sheath?
10 Fr is the inner diameter, about 3.3 mm, where the scope passes. 12 Fr is the outer diameter, about 4 mm, the part that sits in the ureter.
Which ureteral access sheath size is most common?
10/12 Fr and 12/14 Fr are the sizes most hospitals stock, usually in 35 cm and 45 cm lengths.
Is a ureteral access sheath single use?
Yes. Access sheaths are supplied sterile and are made for one patient and one procedure.
Can sheaths be made in custom sizes?
Yes. Manufacturers such as SPG Medisafe can produce custom diameters, lengths and private-label packs for OEM partners.
